Prescription Information
The PH-36 Psoriasis Lamp is a medical device and requires a doctor's prescription.  We will ship your lamp after we have received your prescription.  Prescriptions can be faxed to us at 800-544-3757.

Insurance Information
A portion of the cost of the lamp may be covered by medical insurance.  All plans are different - check with your insurance carrier before ordering to find out if the PH-36 Psoriasis is a covered item.  We do not participate in insurance processing - your insurance company can help you with what you need to process your claim.  We can provide copies of packing slips, paid invoices, etc. as needed.
